Our qualified Physiotherapists, Occupational Therapists or Speech Therapists will visit your home to find out your rehabilitation needs and tailor suitable treatment plans to aid in your recovery, so that you can return to independence from injury and illnesses such as stroke, heart and lung diseases.

Our doctors are able to perform medical assessments and prescribe appropriate diagnostics and treatment. Consult us for Acute or Chronic conditions, Medical procedures, Lasting Power of Attorney (LPA), Advanced Medical Directives (AMD) or Disability Assessments.

Our trained caregivers will assist you and your loved one's Activities of Daily Living (ADLs) for safe and independent living at home. These include eating, bathing, getting dressed, toileting, transferring and continence.

Our trained nurses can assist with procedures such as Nasogastric Tube (NGT) Feeding, Urinary Catheter change, Administration of Medications/ Injections, Blood Glucose Check and many more.
About Us
Dr. Niccol Lee,
Founder of Heart & Hearth
Dr. Niccol Lee established the company with a deep desire to bring compassion, high quality home care to the community. As a dedicated Family Physician with a strong interest in Geriatric care, Dr. Lee believes that care delivered at home should be as dignified, attentive and personalised as the care offered in any medical institution.
Dr. Lee holds a MBBS from the National University of Singapore (NUS) and is a Member of the Royal College of Surgeons (MRCS Edinburgh). He completed the Graduate Diploma in Family Medicine (GDFM) , Graduate Diploma in Geriatric Medicine (GDGRM) and Graduate Diploma in Mental Health (GDMH), equipping him with the clinical breadth and depth required to care for older adults holistically.
Dr. Lee served as the attending physician for Jamiyah Nursing Home and Orange Valley Nursing Home, where he gained extensive experience in managing complex geriatric needs. His commitment to excellence has been recognised through several accolades, including:

- Book Prize Winner of Graduate Diploma Geriatric Medicine
